Ouch. This is a good reminder to check that the barrel is clear, Often.
Folks ain't got a sense of humor no more. They don't laugh they just get sore.

Need to follow the rules. Just hard to determine which set of rules to follow
Now the rules have changed again.

Are you guys sure that was an A-Bolt?
 I know some Sakos were recently  recalled for the barrels doing that on a regular basis- bad batch of blank steel.  They weren't obstructed either, just normal shooting with factory loads...

Do you usually put a piece of tape over your muzzle?  I never have.  Just always careful with my muzzle and keep it out of the dirt etc.

its a great measure of insurance on a hunt like that, its a strong recommendation for hunters in alaska, canada, africa etc. to do this before entering the field, to prevent such events from happening.

 I always tape the muzzle while on a hunt, IF I remember to!
   
I've had to make a "cleaning rod" from a willow sapling more than once to clean barrels plugged with mud or snow.
 Taping the muzzle is much faster and easier!   Thunbs Up

I've shot 100 yard groups with the muzzle taped, (one layer plastic electrical tape) with my 30-06, and there was no discernable change in POI vs. the untaped muzzle.
 
 ...If anyone is worried about that being an issue...
